Cambridge Dental Center ("we," "us," or "our") is committed to protecting the privacy of your health information. We are required to follow the terms of the Notice currently in effect.
We may use and disclose your health information to provide, coordinate, or manage your dental care and related services, including sharing information with other health care providers involved in your treatment.
We may use and disclose your health information to obtain payment for the services we provide, such as billing and collecting payment from you or your dental or medical benefit plan.
We may use and disclose your health information for our internal operations, including quality assessment, staff training and evaluation, licensing, and general business management.
We may contact you to provide appointment reminders or to tell you about treatment options, services, or other health-related benefits that may be of interest to you.
We may use or disclose your health information when required to do so by federal, state, or local law, and for public health activities, health oversight, judicial and law enforcement purposes, and other uses and disclosures permitted or required by law.
Other uses and disclosures of your health information not described in this Notice will be made only with your written authorization. This includes most uses and disclosures of psychotherapy notes, uses and disclosures for marketing purposes, and disclosures that constitute a sale of PHI. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.

We reserve the right to change this Notice and to make the revised Notice effective for health information we already have as well as any information we receive in the future. The current Notice will be posted in our office and on our website, and will show the effective date.
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
